As promised, I upgraded from MovableType 2.21 to 2.63, but the Category problem didn't go away. I got some bad error messages, and in the end, convinced that my Berkeley database was corrupted, I exported all my entries, stored all my templates, copied all my subscriber lists etc and reinstalled MT from scratch... Bit of a pain, and you will see that for a while things will be a bit messy in the archive-department (I'm trying to get the Related Entries working properly, as well as I'll go though the archive and assign proper categories to the entries, a few at a time)...
I got similar problems when I moved my site to a new host -- the categories wouldn't display and related entries weren't generated. I fixed the problem setting the entry_primary_category field in the mt_entry table to null in all entries and changing line 19 in RelatedEntries.pl to:

I don't know if this will help as your former MT installation didn't run against MySQL as mine did.
